Center for sight will submit a claim to your medical insurance for exams with a medical diagnosis. We also participate with select vision plans and will submit a claim on your behalf for routine eye exams.

Affordable Payment Options
Financing options are available for LASIK surgery, premium cataract surgery and varying cosmetic and oculoplastic procedures. Our goal is to make procedures affordable at any budget! Ask about financing options during your appointment or call 850-476-9236.

Insurance
Center for Sight participates in numerous insurance plans; however, it is the responsibility of the patient to review his/her insurance coverage and to know if a referral or pre-authorization is required. Co-payment is due at the time of service.

Insurance Claims
Center for Sight will submit a claim to your insurance carrier, provided we have all the necessary information. This includes a copy of your insurance cards (primary and secondary). Patients are responsible for any deductibles, co-pays, co-insurances, and non-covered services. Any disputes regarding patient responsibility should be directed to your insurance company.
